Plaque Order Form

We offer you the opportunity to purchase brass plates for Memorial/honoraria to be attached to the top rear of pews in our sanctuary. This is another way to remember and/or honor your family and friends with an engraved plaque.

Each plaque measures 3 ½ ” x 1 ¼ “
There is space for four (4) plaques per pew.
Each plaque will cost $125.00 with three (3) lines of engraving.
